To parents of Grade 6 students,The public health nurse for our family of schools dropped off individualized forms for each grade 6 student regarding their immunization history and what shots they will need this year.
The consent form needs to be returned by next Thursday November 21st. The immunization date is set for December 3rd.
If you choose to opt out of these immunization, please send along a note so I know not to expect your forms and to inform our nurse :)
